RCPA QAP KIMMS Workshop

Workshop Objective: To develop a national data set to enable participants to measure and monitor pathology incidents Development of a National Data Set Prior to work commencing on software development to extract KIMMS data from Laboratory Information Systems, the data set needs to be finalised. The KIMMS 2007 Pilot has identified the need to improve and clarify key definitions and the data set, to enable standardised reporting. The changes are reflected in the 2008 KIMMS Data Collection Form

This workshop will facilitate discussion amongst participants on;
- Key definitions e.g. mis-identification, actual /potential harm
- Collecting/extracting data
- Over-reporting/under-reporting
- Sample rejections
- Classification of laboratories
- Sampling (1 week/3 months)
- Protected Quality Assurance Activity

AIMS Workshop (2 hrs) Part 1: 10min
- Welcome brief background; QAA status; QUPP funding of project
- Introduction include some problems encountered to date (over, under-reporting, data collection difficulties)

Part 2: 30min

- Incident
- Actual/potential harm
- Internal/External collections
- Classification of laboratories

Part 3: 30min
Pre-analytical categories
- Identification problems &
- Sample rejections
Post-analytical categories
- Results retracted due to error by laboratory
- Results sent to wrong doctor

Part 4: 40min Incident identification & investigation
- Identification of incident
- Outcome of incident
- Root cause of incident
- Overall incident rate

Part 5: 10min Closing
